  • Emergency Medicine Pharmacist
  • Medication evaluation
  • Reviews new medication orders for appropriateness of drug, dose, route, frequency
  • Intervenes to recommend improvements in drug therapy and documents on intervention documentation form
  • Perform medication reconciliation via gathering patient medication history and supervision of pharmacy technicians obtaining medication histories for patients being admitted to the hospital
  • Follows clinical guidelines e.g. therapeutic substitution, renal dosing, QT prolongation, IV to PO substitution, automatic adjustment of medications
  • Effectively participates in medication utilization reviews
  • Completes adverse drug reaction reports, retrospectively and prospectively
  • Specialize in providing Emergency Department pharmacy services weight, age, potential drug interactions or incompatibilities, allergies, and cost effectiveness
  • Enters/verifies, compounds and dispenses medications and IV’s as needed in a timely manner while controlling waste
  • Supervises technicians and coordinates their functions in product preparation, distribution, labeling and documentation
  • Provides factual information about medications in a timely manner to patients, health care professionals or other customers
  • Delegates appropriately to professional and supportive personnel, and supervises their pharmacy technician activities

Education

  • B.S. or Pharm D from an accredited college of pharmacy

Qualification/Skills

Experience

  • Hospital pharmacy experience with additional training/experience with emergency medicine preferred

Skills

  • Therapeutic drug monitoring, Unit Dose dispensing, I.V. Admixture, medication compounding
  • Teamwork and effective oral and written communication skills required
  • Will contribute to learning of students, interns and residents as part of day to day work assignment. Will contribute to orientation and training of new staff as applicable
  • Computer skills with proficiency in MS Office products are preferred
  • Maintain ACLS and BLS training and certification status
  • Patient counseling skills
  • Ability to work collaboratively with physicians and nurses in the emergency department
  • Ability to adapt to a rapidly changing environment
  • Ability to directly supervise and delegate pharmacy technician responsibilities

Licensure/Certification

  • Valid license to practice pharmacy in the state of Minnesota
  • ACLS and BLS training and certification preferred

Work Schedule

7 - 10 hour days on/ 7 days off

The city of Coon Rapids is a fast growing community on the Mississippi River just 20 minutes north of Minneapolis-St. Paul, and the metropolitan area gateway to the northern part of the state. Coon Rapids offers excellent access to highways, dynamic community shopping centers with major retail anchors, and a wide variety of recreational activities including neighborhood and regional parks, a municipal golf course, indoor ice arenas, hiking and biking trails and swimming pools.
