  • Transition and support patients through all stages of care
  • Maintain and document ongoing communications (via phone, in-person, and EHR) with patients and care team
  • Collect and maintain patient data and reports to facilitate care
  • Communicate with patients, families, and providers to maintain coordination of care
  • Adhere to scope of practice and refer any medical questions to appropriate healthcare personnel
  • Identify barriers the patient may have to meeting health care goals and communicate with care team
  • Assist patient in determining resources within Allina Health and the community
  • Connect patients to appropriate services and resources
  • Follow-up with patients to ensure connections were made and ongoing needs are met
  • Work with patients to develop medical and non-medical goals
  • Reinforce provider and care team communication about plan of care
  • Engage family and/or friends (as requested by patients) to support care plan and goals
  • Support medication and treatment adherence, if applicable
  • Document in EHR according to department procedures and workflows
  • Access and maintain patient panel
  • Utilize standard epic tools and smartphrases
  • Provide relevant and timely updates to the care team according to department guidelines
  • Document patient personal and care goals and progress
  • Use behavior change techniques to empower patients to be active participants in their health
  • Incorporate techniques into daily practice with patients
  • Use techniques to support the plan of care
  • Provide behavior change mentorship to other team members or care guides, as needed
  • Prepare weekly team huddles by supplying team with patient names for discussion
  • Expert in Motivational Interviewing (MI) techniques


  • High School Diploma required; Bachelors degree preferred
  • Valid MN driver’s license and reliable transportation, if required by department



  • Two years’ experience in a customer service, healthcare, or community-based role
  • Ability to multitask, prioritize and manage competing demands
  • Ability to build and maintain a relationship with the patient independently
  • Ability to demonstrate critical thinking
  • Strong interpersonal skills and ability to work collaboratively
  • Adaptable and comfortable with ambiguity
  • Comfort, sensitivity, competence and experience serving people of various cultural and socioeconomic backgrounds
  • Excellent verbal and written communication skills
  • Demonstrates expertise in communicating with various levels of professionals
  • Ability to learn and utilize behavior change techniques (i.e. coaching, Motivational Interviewing)
  • Experience working autonomously and as a member of a team
  • Willing to help people without enabling or encouraging dependency
  • Elicits cooperation while acknowledging divergent viewpoints
  • Demonstrates integrity, discretion and understanding of professional boundaries
  • Ability to manage conflict that may occur with patients and family dynamics
  • Interested in healthcare; can quickly learn medical and pharmacological terminology
  • Proficiency with commonly utilized technology


  • Experience working within health care or other complex systems
  • Knowledge of medical or pharmacological terminology

Work Schedule

  • This position will be based out of Golden Valley within our Courage Kenny location. There will be occasional travel to Abbott Northwestern and the Allina commons for meetings.
  • Day to day work schedule will be 8:00am-5:00pm, normal business hours.
