Responsibilities

Responsible for the development of programs, provision of direct patient care and coordination of department activities to provide optimum pharmaceutical care services for patients and customers.
Patient care responsibilities include a combination of Antimicrobial Stewardship and Critical Care support in addition to other duties as assigned.
The position is also responsible for supporting front line pharmacists, department leaders, nursing staff and providers by developing programs and education to advance the practice of pharmacy and improve clinical outcomes.

Additional responsibilities may include but are not limited to:
  • Conducting Medication Use Evaluations and Drug Formulary Reviews and implementing improvements
  • Precepting residents and students
  • Collaborating in quality and safety initiatives that will enhance the care provided to patients
  • Ensuring regulatory requirements are achieved in collaboration with other leaders
  • Supporting the Department of Pharmacy to achieve financial performance goals, strategic initiatives and practice advancement
  • Other site or system committee support as assigned.

Education

Graduate of an accredited College of Pharmacy with a Doctor of Pharmacy (PharmD) degree.


Qualification/Skills

ASHP-accredited PGY-1 Residency required.
ASHP-accredited PGY-2 Residency or similar experience required with area of focus in Infectious Diseases or Critical Care preferred
Pharmacist Board Certification is required: Infectious Diseases or Critical Care Preferred.
