Responsibilities

  • Provide effective care coordination, assessment and oversight across a continuum of services and settings for the high risk obstetric population with a focus on fetal anomalies
  • Responsible for following standards of care, policies and procedures as established within Allina and the Midwest Fetal Care Center
  • Serve as a patient/family advocate
  • Serve as a role model in promoting the interdisciplinary team to ensure comprehensive coordination of care
  • Participates in the facilitation of weekly huddles, rounding and case presentations
  • Works collaboratively within the fetal team tin development of clinic care pathways
  • Facilitates chart prep, accessing and ensuring pertinent records are available for visits
  • Triage patient care needs while in clinic and via telephone encounters
  • Assess learning readiness based on cultural, age and education level
  • Update and create patient EMR to accurately reflect plan of care
  • Provide regular updates to referring providers and sub specialists
  • Assist and participate with ongoing data collections, research and QA projects
  • Effectively engage/communicate and or lead a multidisciplinary team in a group setting, including planning, coordination of monthly multi-disciplinary fetal care meetings
  • Promoting effective utilization of health care and monitoring of outcomes based on the needs and input of all stakeholders

Education

  • Registered Nurse with Bachelors or Associate degree

Qualification/Skills

Experience

  • Prefer two years experience working with high risk obstetric patients
  • Knowledge of fetal anomalies
Skills
  • Proven exceptional communication skills both verbally and written
  • Proven ability to assess and support all obstetric conditions, normal to abnormal
  • Proven ability to establish and develop rapport with patients and families, and a multidisciplinary team
  • Desire to work collaboratively and facilitate professional relationships
  • Skilled at multi tasking
  • Critical thinking and creative problem solving skills
  • Exceptional customer service skills
  • Adaptable to an unpredictable clinic setting
  • Ability to work well without supervision
Licensure/Certification
  • Registered Nurse—Minnesota

Work Schedule

  • Casual position - must be available a minimum of 4 shifts a month.
  • Shifts begin between 7-8 am
  • Non contract position
  • Work is generally done at main locations at Abbott and United
  • Care coordination staff wear business casual, no jeans.
