Allina Health Home Care Services provides skilled medical care for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 30 Minnesota counties, 24 hours a day, seven days a week.

Responsibilities

  • Assess, develop and coordinate and/or deliver the spiritual care plan
  • Orientation and training of new staff and volunteers
  • Attend monthly spiritual care meetings
  • Participate in committee work
  • Attend spiritual development meetings
  • Participate in on-call responsibilities as scheduled
  • Participate on Hospice Care Team
  • Communicate changes in status effectively to team members, patient, and families
  • Document all visits and phone contacts
  • Provide casual coverage for full time staff when they are on PTO, sick, or as assigned.

Education

  • Masters of Divinity or equivalent
  • Clinical Pastoral Education (4 units)
  • Ordination preferred
  • ACPE Board Certification Eligible

Qualification/Skills

Experience

  • Healthcare or Hospice Spiritual Care experience preferred

Skills

  • Ability to respond to the spiritual care issues of the dying patient and family
  • Ability to work autonomously and manage a caseload in a high activity environment
  • Knowledge about & comfort with a variety of spiritual beliefs
  • Sensitivity and ability to provide compassionate and caring presence to patients and families with varied lifestyles
  • Ability to collaborate with community clergy and other team members
  • Competency with a laptop computer, an automated medical record, care plans and medical standards documentation

Licensure/Certifications

  • Minnesota Driver’s License
