Responsibilities

  • Provides the expertise in areas relating to social service
  • Demonstrates skills and knowledge to meet the physical, psychological aspect of all age groups
  • Evaluates patient and family information, selects appropriate social work methods, and develops, implements and evaluates a plan in conjunction with the interdisciplinary efforts of the care team
  • Actively participates and provides pertinent information in discharge planning meetings
  • Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement
  • Consults, assesses, refers, coordinates interventions in cases of suspected or actual situations of abuse, neglect or domestic violence
  • Serves as the lead in obtaining legal guardianship and competency determinations
  • Completes and disseminates all necessary legal and clinical documentation as required
  • Serves as the lead in managing adoption related situations
  • Works with patients and family members to listen to concerns and diffuse difficult situations
  • Works with department managers to resolve patient concerns
  • Works with physicians to resolve patient concerns
  • Appropriately documents and communicates all patient concerns and compliments
  • Utilize tools and resources within Allina in order to efficiently document and organize patient and family concerns
  • Involves appropriate managers/staff/physicians in resolving concerns

Education

  • Masters in Social Work preferred
  • Bachelor Social Work degree accredited by the council on Social Work Education required

Qualification/Skills

Experience

  • Minimum of one year experience in acute care preferred
  • Additional training in acute, long term care and home health
  • Previous experience in staff and community education preferred

Skills

  • Good communication skills
  • Knowledge of adult learning principles
  • Basic computer skills preferred

Licensure/Certifications

  • Certified or eligible for Wisconsin Social Work certification

Work Schedule

Everyone other weekend SAT and SUN day hours 8-4:30
